PURPOSE: To study the efficacy of the tenectomy of the posterior fibers of the tendon of superior oblique near the scleral insertion, a selective technique that weakens only the fibers, vith main vertical and horizontal action, preserving the torsional fibers, as proposed by Prieto-Diaz. METHODS: Twenty eight patients had the measurements made and were operated on by the same surgeon (KMC) for the correction of A-pattern anisotropia being 23 associated with eso and 5 with exotropia. Were selected the preoperative incomitance varying from 10-25", with no previous surgery. The surgery made utilized the technique described above and correction of horizontal deviation. RESULTS: The average preoperative incomitance was 17,10Δ with standard deviation of 4,97Δ. The average final incomitance was 5,21Δ with standard deviation of 6,83Δ. The absolute amount of correction was 11,9Δ with standard deviation of 7,29Δ and the percentual was 70,35% with p < 0,001. CONCLUSION: The posterior tenectomy of superior oblique did a significant decrease of the A-pattern anisotropia being the absolute value of correction more important for the clinical purposes. The efficacy of this surgery was considered good since the analysis of significance of the coefficient of correlation and the regression line showed significance.
